Foundation for Surgical Technology
The purpose of the scholarship is to encourage and reward educational excellence as well as respond to the financial need demonstrated by the surgical technology student and offer assistance to those who seek a career in surgical technology.
Scholarship recipients are seleted on the basis of academic excellence and financial need combined.
Applicants must provde an official course fee schedule from their educational institution, in addition to official transcripts, recommendation and mentor report.
Scholarshipships are awarded based on the informatio provided on the application. Applicants must be enrolled in a CAAHEP-accredited program of surgical technology in order to be eligible. In addition, applicants must demonstrate superior academic ability and have a need for financial assistance.
1. A complete application consists of four parts:
Part 1: Student Responsibility: Click here for the FST Scholarship application and return the forms to your instructor with a stamped envelope.
Part 2: Official Transcript: ONLY official transcripts will be accepted.
Part 3: Instructor Evaluation. This form is to be completed by the program director or designee. Fill out the interactive form and include it with the application.
Part 4: Mentor Section: To be completed by individual in a related health care field (eg clinical preceptor). Complete interactive form.
2. Applications must be completed and turned in to the instructor. Students must provide a stamped envelope
3. The instructor must complete Part 3, the Instructor Evaluation and submit it with the completed application to: Scholarship Department, The Foundation for Surgical
Technology, 6 West Dry Creek Circle, Littleton, CO 80120.
4. Incomplete or late applications will not be accepted and will not be returned.
ALL APPLICATIONS MUST BE POSTMARKED BY March 1, 2011.
(Applications must be mailed by instructor. Students must provide the instructor with stamped, preaddressed envelope) Do not combine this application with any other application.
Scholarships will be awarded with the amount determined by the Foundation committee on the basis of merit and financial need. Scholarship winners will be notified at the beginning of the AST Annual National Conference. Each scholarship recipient will be notified by mail. It is the responsibility of each recipient to provide a letter of personal goals and a picture for publication in order to receive the award. The names of the winners are published in the Journal and on the Foundation and AST websites.
Scholarship funds are provided entirely through contributions to the Foundation by individuals, AST constituent divisions, corporations and others who desire to support the program of the Foundation.
